Patient Assessment Standards Coordinator - Full Time Days / 32 Hour Per Week - 8 hour days / Mon, Tues, Wed & Friday

Fairlawn Rehabilitation Hospital, an affiliate of Encompass Health is a 110 bed adult acute rehab hospital in Worcester, MA. We are Joint Commission Accredited and Gold Seal Certified as a Stroke, Parkinson’s, Amputee and Brain Injury Center of Excellence.

Using discretion and independent judgment the Patient Assessment Standards Coordinator (PASC) ensures compliance by all responsible parties throughout the organization, as it relates to capturing patient gains on their journey from hospital to home. Section GG in the universal coding/documentation language used across healthcare providers to record a patient’s progress. It replaces the former MDS/FIM tools.

 

From pre-admission to discharge you will monitor that critical assessments, evaluations and authorizations are in place for every patient we provide services for. Accurate tracking and coding of functional improvements using GG are critical to assessing clinical outcomes and securing correct reimbursement for our services.

 

70% of your time will be on the floor working with doctors, nurses, and therapists and 30% in-office.

 

Working with our established electronic medical records system and hardcopy documentation you will review patient records for compliance to numerous time sensitive transactions. The PASC will help to create new and utilize existing tools to track a wide variety of actions that occur during a patient’s stay supporting our commitment to 100% compliance in all regulatory categories. Our ability to correctly capture a patient’s progress is key to our business and to receive best reimbursement for the great work that we are doing.

 

This position requires a unique mix of office based analytical/organizational skills and primarily on the floor teaching and face to face follow-up with a range of medical professionals including hospital administration, physicians, nursing management, case managers and staff nurses. The position requires the development and maintenance of positive working relationships, as well as serving as an educator to the nursing, therapy and the physician staff working within the hospital.

 

In addition, this position will help to develop hospital wide Section GG policies/procedures and act as the primary resource for immediate problem solving in regards to complying with these guidelines.

Requirements

Registered Nurse or LPN or Licensed Occupational or Physical Therapist

 

Previous experience in Risk Management, Utilization Review, Quality Control, helpful, but not required.

5 plus years of acute care or medically complex patient care experience ideal

Strong skills in Excel and other Microsoft Office products.
