Encompass Health is growing and we are looking for a compassionate, talented and experienced Registered Nurse(RN) to join our Amazing Team!

The Clinical Care Coordinator - Registered Nurse (RN) has primary responsibility for ensuring a strong and stable relationship between the agency and one or more assisted living, independent living, and long term care facilities. The Clinical Care Coordinator - Registered Nurse (RN) acts as the voice for Encompass and pro-actively addresses all patient care issues and challenges at the facility level. The Clinical Care Coordinator - Registered Nurse (RN) administers skilled care to patients requiring professional services and teaches the family and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the patient’s physician.


The Clinical Care Coordinator - Registered Nurse (RN) must meet the following qualifications:

  • Be a graduate of an approved school of nursing and be licensed as a Registered Nurse by the state in which he/she practices
  • Have a minimum of one year of clinical experience
  • Have a minimum of one year of hospice experience
  • Must have a demonstrated knowledge and understanding of the issues related to the delivery of hospice care and the working environment of an assisted/independent living facility and/or long term care facility
  • Be organized, detail-oriented, and possess effective written and oral communication skills
  • Be a service-minded individual with the ability to work effectively with a wide range of personalities and disciplines
