Encompass Health is searching for an experienced medical scheduler to join our team as a Patient Services Coordinator (scheduler)  & Intake Coordinator.  


This is a hybrid position responsible for scheduling patients and managing all aspects of the patient intake process to ensure hospice admissions are completed in an organized and timely manner. The role will provide direct support to the Branch Director and will be cross-trained in several areas of office administration – Medical Records, Intake, Billing, Scheduling, Human Resources, and Payroll.


Patient Scheduling Responsibilities:

  • Schedule home visits for the field staff each day and assisting with the coordination of patient care.
  • Ensure geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care.

Intake Responsibilities:

  • Accept incoming referrals from physicians, facilities, and sales managers and enter into the EMR (Homecare Homebase).
  • Act as a liaison among staff, patients, family, physicians, referral sources and payers. 
  • Maintain close communication with the sales team, clinical team, referral source and patient during the referral/admission process.
  • Enter Non-Admit information into EMR with details if no visit was made and communicate with referral sources and/or Sales Managers along with the reason for Non-Admission.

Hours: Monday - Friday, 8:00am - 5:00pm

The ideal candidate for this role will be an LPN that excels in a fast paced environment with exceptional customer services skills.

  • LPN preferred but not required.
  • At least 1 year of patient scheduling experience is required.
  • A minimum of 1 year of home health or hospice experience is strongly preferred.
  • Strong understanding of staffing and scheduling requirements related to home health and hospice.
  • Previous experience working with Homecare Homebase (HCHB) is preferred.
