Hours/Schedule: Monday 5:00pm – Friday 8:00am (after hours / during office closure hours).    

 

Monday 5pm to Tuesday 8am

Tuesday 5pm to Wednesday 8am

Wednesday 5pm to Thursday 8am

Thursday 5pm to Friday 8am

Responsibilities

The On-Call Hospice Triage Registered Nurse (RN) is responsible for:

  • After-hours on-call triaging of patient calls. Dispatch clinicians as needed. 
  • Ensuring that care and services are delivered appropriately and timely during after-hour and holiday coverage.
  • Will directly affect employee, family & referral source satisfaction.
  • Working to reduce unnecessary hospitalizations and Emergency Department visits during after-hours and holidays.
  • Reviewing and processing weekend / Holiday referrals and schedule the admissions to local on-call staff.

After-hours is defined as Monday at 5 PM – Friday at 8 AM (after hours / during office closure hours).   

Qualifications

  • Must be a Registered Nurse (RN) and have AL licensure.
  • Must have a minimum of five years full-time experience as a RN.
  • Must maintain internet services in their home office.
  • Must be able to work Monday 5:00pm - Friday 8:00am and on Holidays.
  • Must have a minimum of two years full-time experience in hospice nursing. 
  • Must have one year of full-time recent experience in a supervisory hospice RN position.
  • Must have an excellent understanding of the Federal, State and local laws/regulatory guidelines governing the operation of a Home Health / Hospice agency.
  • Must be professional, organized and able to effectively communicate both orally and in writing as well as prepare and present education material to Agency employees and community referral sources utilizing computers/electronic equipment.
  • Must be capable of multitasking while maintaining a professional and friendly demeanor.
