Responsibilities

1.Perform patient visits in a timely, professional, and appropriate manner per standards of care.Ensure in-home visit time fully meets the needs of the patient.
2. Record and report the patient’s reaction to the therapy program including any changes in the
patient condition to the licensed professional.
3. Coordinate total patient care including interdisciplinary communication with other health care
providers, office, and physician.
4. Utilize various types of therapy equipment as established by the supervising licensed professional.
5. Guide and instruct patient and their families in prescribed therapeutic activities that are directed
toward improving independence and functionality.
6. Cooperate and assist other health care providers to enhance continuity of care.
7. Ensure that missed patient visits are rescheduled in the same Medicare week in order to adhere to
the patient’s POC.

Qualifications

Education and Experience (ESSENTIAL):

  • Must be a graduate of an approved school of clinical education.
  • Must be licensed in the state where they currently practice.
  • Must have at least one year experience as a licensed therapy assistant.


Education and Experience (DESIRED):

  • Experience as a licensed therapy assistant may be deferred with a review and approval from the regional vice president for operations or regional director.
  • Medicare home health or hospice experience is preferred.

 

Qualifications:

  • Must be organized and detail oriented.
  • Must be able to communicate effectively both orally and in writing.
  • Must understand the issues related to the delivery of home health or hospice care and be able to problem-solve effectively.
  • Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
  • Must function well in atmosphere of stress and interruption.

 

Requirements:

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider
